Medicaid Service Coordination
Service coordination assists persons with developmental disabilities and their families in gaining access to services and supports appropriate to their needs. MSC is provided by qualified service coordinators and uses a person-centered planning process in developing, implementing, and maintaining an Individualized Service Plan.
Services
- Assistance with securing entitlements and benefits such as SSI, food stamps, Medicaid, Medicare, etc.
- Offer advocacy supports for Individualized Educational Plans (IEP) in the school settings.
- Develop and maintain the consumer's Individualized Service Plan (ISP) for waiver services.
- Provide oversight and monitoring of all services received by the individual.
- Assistance with accessing programs and funding sources for environmental modifications and assistive technology.
- Crisis intervention and support.
Requirements:
Individuals and family members are required to meet face-to-face with the Medicaid Service Coordinator at least three times per year to maintain this service. Once a year, one of the face-to-face visits is required to occur in the home. A review of the ISP must occur at least twice annually.
Plan of Care Support Services
PCSS is an alternative form of service coordination. PCSS providers assist individuals to review and update their Individualized Service Plan. PCSS is delivered by a qualified Medicaid Service Coordinator. Typically, an individual must have received at least three months of Medicaid Service Coordination to be eligible for PCSS, with the exception of Early Intervention enrollees.
Family Reimbursement
This service provides financial assistance/reimbursement to developmentally disabled individuals and/or their families that care for them. Specifically, the family reimbursement funds assist families with designing, obtaining, and implementing services to meet their family memeber's needs. Families are able to utilize these funds to lessen the financial strains they are experiencing.
Eligibility:
To qualify for both Medicaid Service Coordination and Plan of Care Services, an individual must have a documented diagnosis of a developmental disability as defined by the NYS Office For People With Developmental Disabilities (OPWDD). The onset of the disability must occur prior to the age of 22.
Cost:
None to Medicaid eligible individuals.
